圖書證網上申請服務

ONLINE APPLICATION FOR LIBRARY CARD (BORROWER CARD)

凡本校教職員工和學生均可進入圖書館外借書籍或多媒體資料。其他人士（年滿十八歲者）基於研究上的需要，可透過此網上平台或親臨方式向本館申請借書證。申請手續及申請資格請瀏覽圖書館網頁。
All academics, students and staff of MPU can access our Libraries. External patrons aged above 18 are also welcome but a Borrower's Card is needed for using the Library or borrowing any library materials. The application can be made through this online platform or submit in person at Library, please visit the Library website for application or condition details.

網上申請程序 ONLINE APPLICATION PROCEDURES

1. 填妥以下網上申請表格；
   Complete the online application form below;
2. 上載申請人的近照及所需身份證明文件副本之電子檔案。
   Upload the digital copy of your recent photo and the identity document(s) that is/are required for the application;
3. 申請在提交並獲圖書館審核後將發出領證通知電郵，申請人可自該電郵發出起計90日內按電郵指示親臨圖書館完成辦理手續。屆時申請人須出示相關身份證明文件正本以供核對，並在申請表上簽署。如逾時未完成手續，相關申請記錄將被刪除，申請人須重新遞交申請。
   After submission of application and being approved by the Library, a notification email would be sent to the applicant. The applicant is required to follow the instructions given to complete the application procedures at Library in person within 90 days from receiving the email. And the applicant must present the original copy of the identity document(s) used for the application while the applicant should sign on the application form at the Library. If the procedures are not completed within the time mentioned, the relevant application records will be deleted and the applicant must resubmit the application.
4. 保證金可以使用線上繳付，該保證金在申請人取消借書證時發還
   The deposit can be paid online and will be refunded upon the cancellation of the library card.

申請須知 NOTES ON APPLICATION

1. 此網上平台只供從未領有本館圖書證的人士辨理申請；如續期、換領、報失或取消圖書證，請親臨圖書館櫃檯辦理有關手續。
   This online platform is provided for the applicant who has never applied for a MPU Library Card to submit the application. For renewal, replacement, reporting lost or cancellation, please submit the application in person at Library Circulation Counter.
2. 上載至本平台的文件副本電子檔案是為圖書館職員處理申請時核實之用，在提交申請前請務必核實並確認所輸入的資料及上載的文件正確無誤及為有效，否則申請有可能被延誤或取消。
   The electronic file(s) of document copies uploaded to this platform is/are for verification and processing your application, please make sure the information provided are correct and verified against the uploaded document copies as valid. Otherwise, the application might be delayed or rejected.
3. 申請資料及相關身份證明文件的電子檔案會在完成辦理申請手續後從系統刪除。
   All applicant information and documents, including copies of the proof of identity uploaded to the platform will be deleted after completion of the application procedures.
4. 為減低個人資料洩露的機會，強烈建議你不要在公眾電腦上辦理網上申請手續。如並非使用自己的個人電腦辦理申請手續，請緊記於完成申請程序後，把個人資料及身份證明文件電子檔案徹底從電腦中刪除，並關閉所有視窗。
   Applicants are strongly advised not to submit the applications on public computers in order to minimize the chance of leakage of personal information. If applicants are not using their own personal computers to submit the applications, please be reminded to completely delete all personal data (including the files of the proof of identity) that was saved on the computers and close all browser windows.
5. 申請人須注意，在公開網絡上傳送資料，可能存有被未經許可或授權的第三人看到和使用的風險。倘　閣下對此類風險感到不安，請使用網絡以外的其他方式提交申請。詳情請參閱澳門理工大學網頁私隱政策聲明。
   Applicants should note that the transmission of information on an open network may entail the risk of being seen and used by authorized/unauthorized third parties. If you are uncomfortable with foregoing, please submit the applications via other channels. For details, please refer to the Macau Polytechnic University Web Site Privacy Policy Statement.
6. 如遺失圖書證，必須親往圖書館櫃檯辦理報失手續，在辦理報失手續前可先行致電通知圖書館，以防止失證被盜用。
   Report of loss of library card must be made in person at Library Circulation Counter. The library card holder may, however, first report the loss by telephone to the Library in order to prevent the unauthorized use of the lost card.
7. 個人資料如有更改，請通知澳門理工大學圖書館。詳情請向圖書館職員查詢。
   Any change of personal particulars should be notified to the MPU Library. For details please contact our library staff.
8. 圖書證不得轉讓，申請人須遵守圖書館服務使用指引。
   Library Cards are not transferable. The applicant is required to observe Guidelines for Using MPU Library Services of the Library.
9. 圖書館館長保留拒絶任何申請的權利。
   The Library Director reserves the right not to approve any application.
